Lý do phẫu thuật này được đề xuất
Bác sĩ Kieran Hirpara, bác sĩ phẫu thuật chi trên tại Bệnh viện tư nhân Mater Rockhampton, sẽ bắt đầu bằng các phương pháp ít xâm lấn nhất phù hợp với tình trạng của bạn. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám chúng tôi; nếu vật lý trị liệu viên khuyên bạn nên gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả từ Medicare. Đối với các vấn đề do hao mòn lâu ngày, chúng tôi thường thử các biện pháp điều trị không phẫu thuật trước: thay đổi hoạt động sinh hoạt, vật lý trị liệu hoặc trị liệu tay, cùng việc đeo nẹp. Phẫu thuật chỉ được cân nhắc khi những biện pháp này vẫn chưa giúp ích đáng kể. Mục tiêu của ca phẫu thuật là giảm đau, cải thiện chức năng vận động và bảo vệ các khớp lân cận. Đối với một số người, phẫu thuật còn giúp ổn định lại khớp bị lỏng lẻo hoặc biến dạng.
Trước khi phẫu thuật
Trong những tuần lễ trước khi phẫu thuật, chúng tôi sẽ xác nhận kế hoạch điều trị bằng cách chụp lại hình ảnh bàn tay của bạn, chẳng hạn như X-quang, MRI hoặc siêu âm. Những hình ảnh này giúp chúng tôi xác định tình trạng khớp bị tổn thương và chọn loại implant phù hợp. Bạn sẽ nhận được hướng dẫn cụ thể về việc nhịn ăn: không được ăn hoặc uống gì trong vòng bảy giờ trước khi phẫu thuật. Chúng tôi yêu cầu thời gian nhịn ăn là bảy giờ thay vì ít hơn để có thể sắp xếp ca phẫu thuật sớm hơn nếu danh sách các ca mổ được thực hiện trước giờ. Một số loại thuốc cần phải ngưng sử dụng trước khi phẫu thuật; bác sĩ phẫu thuật sẽ thông báo cho bạn biết loại thuốc nào và thời điểm nào cần ngưng. Nếu bạn mắc các bệnh lý khác, có thể bạn sẽ cần làm xét nghiệm máu hoặc được bác sĩ gây mê thăm khám. Vào ngày phẫu thuật
Bạn đến đơn vị tiếp nhận bệnh nhân phẫu thuật của bệnh viện, nơi bạn sẽ được làm thủ tục nhập viện và chuẩn bị cho ca mổ. Bạn sẽ gặp bác sĩ gây mê – người chịu trách nhiệm về việc gây mê và kiểm soát cơn đau trong suốt ca phẫu thuật. Ca phẫu thuật này được thực hiện dưới gây mê toàn thân. Đôi khi người ta còn tiến hành chặn dây thần kinh vùng để giảm đau sau phẫu thuật; bác sĩ gây mê sẽ trao đổi với bạn về vấn đề này vào ngày hôm đó. Sau đó, bạn sẽ được đưa vào phòng mổ để tiến hành ca phẫu thuật.
Khi ca phẫu thuật hoàn tất, bạn sẽ tỉnh dậy tại khu vực hồi sức. Các điều dưỡng sẽ theo dõi sát sao tình trạng của bạn trong lúc thuốc mê dần hết tác dụng. Khi sức khỏe ổn định, tùy thuộc vào loại phẫu thuật và diễn biến hồi phục của bạn, bạn sẽ được chuyển sang khu nội trú hoặc xuất viện ngay trong ngày.
Quy trình phẫu thuật
Bác sĩ phẫu thuật sẽ tạo một đường rạch nhỏ ở mặt sau khớp ngón tay để tiếp cận các bề mặt khớp bị tổn thương. Các đầu xương bị hư hỏng sẽ được cắt bỏ và định hình lại để tạo chỗ cho implant. Loại implant phổ biến nhất là một miếng đệm mềm nằm giữa hai đầu xương, giúp khớp có thể cử động trở lại đồng thời giảm đau. Mục tiêu của phẫu thuật là giảm đau, cải thiện khả năng vận động và bảo vệ các khớp lân cận.
Có nhiều loại implant khác nhau, phù hợp với từng loại khớp và từng tình trạng bệnh. Với một số bệnh nhân, người ta sử dụng miếng đệm silicone mềm; còn với những người khác – đặc biệt là khi các dây chằng xung quanh vẫn khỏe mạnh và các ngón tay bên cạnh hỗ trợ tốt – người ta chọn loại implant cứng hơn có hình dạng tương tự bề mặt khớp tự nhiên. Bác sĩ phẫu thuật sẽ chọn loại implant phù hợp nhất với khớp và tình trạng sức khỏe của bạn.
Sau khi đặt implant vào đúng vị trí, bác sĩ sẽ kiểm tra xem ngón tay có thẳng hàng và cử động trơn tru không, đồng thời điều chỉnh các mô mềm xung quanh khớp để đảm bảo sự ổn định. Sau đó, vết mổ được khâu lại và băng vết thương được đặt lên trên. Băng này sẽ được giữ nguyên trong khoảng 10 ngày; phần “Sau phẫu thuật” sẽ giải thích chi tiết hơn về điều này.
Đối với một số khớp ngón tay cái, việc nối liền hai đầu xương lại với nhau (gọi là phương pháp hợp nhất xương) là một lựa chọn thay thế cho việc đặt implant. Phương pháp này giúp loại bỏ cơn đau ở khớp nhưng khiến phần ngón tay cái không còn khả năng cử động được nữa. Sau khi phẫu thuật
Bạn sẽ tỉnh dậy tại phòng hồi sức, nơi các điều dưỡng sẽ theo dõi sát sao tình trạng của bạn khi thuốc gây mê hết tác dụng. Bàn tay của bạn sẽ được băng bó cẩn thận; chúng tôi cũng sẽ cung cấp thuốc giảm đau để giúp bạn cảm thấy thoải mái hơn. Trong 24 giờ đầu sau khi về nhà, nên có người ở bên cạnh bạn. Bạn có thể di chuyển bình thường, nhưng hãy chậm rãi và nghỉ ngơi; khi ngồi, hãy đặt tay lên gối để nâng cao lên. Đội ngũ y tế sẽ thông báo cho bạn biết là bạn có thể về nhà ngay hay phải ở lại bệnh viện qua đêm. Ban đầu, tay bạn sẽ được băng bó cẩn thận; lớp băng này sẽ được giữ lại khoảng 10 ngày. Khi chúng tôi khám lại cho bạn, chúng tôi sẽ thay hoặc tháo lớp băng này và kiểm tra tình trạng lành da. Sau phẫu thuật, bạn sẽ tham gia các bài trị liệu tay dưới sự hướng dẫn của Ruby Doolan tại trung tâm Extend Rehabilitation. Cô ấy sẽ hướng dẫn bạn thực hiện các bài tập và chế tạo nẹp nếu cần thiết. Khi bạn có thể nắm chặt và kẹp vật mà không còn đau đớn, các hoạt động thường ngày như mặc quần áo, ăn uống và viết lách sẽ trở nên tự nhiên hơn. Nhiều người nhận thấy lợi ích lớn nhất là giảm đau so với trước khi phẫu thuật, đồng thời tay cũng được sử dụng hiệu quả hơn trong sinh hoạt hàng ngày.
Bạn không nên lái xe khi tay vẫn đang đeo nẹp hoặc khi đang dùng các loại thuốc giảm đau mạnh; đồng thời bạn cần đảm bảo có thể cầm vô lăng bằng cả hai tay và phản ứng nhanh trong trường hợp phải phanh gấp. Những biến chứng có thể xảy ra
Hầu hết bệnh nhân đều hồi phục tốt, nhưng đôi khi vẫn có thể gặp phải các vấn đề. Bác sĩ phẫu thuật và đội ngũ y tế sẽ theo dõi sát sao để phát hiện sớm bất kỳ bất thường nào.
Nhiễm trùng sau phẫu thuật thay khớp loại này là tình trạng hiếm gặp. Bạn hãy chú ý quan sát vùng da quanh vết mổ. Nếu thấy vùng da đỏ lan rộng ra từ vết mổ, có cảm giác nóng, chảy dịch, hoặc đau âm ỉ sâu bên trong mà thuốc giảm đau thông thường không làm dịu, hãy liên hệ ngay với phòng khám. Nếu có dấu hiệu sốt hoặc cảm thấy mệt mỏi, hãy đến khoa cấp cứu.
Đôi khi khớp nhân tạo có thể bị trật khớp. Bạn có thể cảm nhận được sự thay đổi đột ngột ở khớp; ngón tay hoặc ngón cái sẽ ở một góc lạ và không thể cử động bình thường. Nếu gặp tình trạng này, hãy gọi ngay cho phòng khám.
Theo thời gian, khớp nhân tạo có thể bị mòn, lỏng lẻo, nứt hoặc cong vênh. Các dấu hiệu bao gồm cơn đau quay trở lại sau khi đã giảm, tiếng kêu lục cục hoặc tiếng cọ xát mới xuất hiện, hoặc ngón tay bị lệch khỏi vị trí bình thường. Khi nào nên liên hệ với chúng tôi

Advanced reading: the deeper science (optional)

Silicone không bị dịch chuyển
Một bài tổng quan năm 2026 về phẫu thuật thay khớp ngón tay ở bệnh nhân viêm khớp dạng thấp cho thấy rằng phương pháp này vẫn đóng vai trò quan trọng trong điều trị các vấn đề về bàn tay do viêm khớp dạng thấp; đồng thời dù công nghệ cấy ghép đã có nhiều tiến bộ, phẫu thuật thay khớp bằng silicone mềm vẫn mang lại cải thiện đáng kể về chức năng và thẩm mỹ ở những bệnh nhân được chọn lựa phù hợp [1].
Sau 60 năm nghiên cứu về khoa học vật liệu, đã có nhiều loại vật liệu thay thế như kim loại – nhựa và pyrocarbon; tuy nhiên miếng đệm silicone vẫn được coi là tiêu chuẩn tham chiếu. Điều này khá hiếm gặp trong lĩnh vực thay khớp, bởi trong cùng khoảng thời gian đó các loại implant thay khớp hông và khớe đều được cải tiến liên tục.
Lý do là vì loại implant này không thực hiện chức năng như tên gọi của nó: nó không tạo bề mặt mới cho khớp nhằm phục hồi khả năng vận động. Kết quả điều trị phụ thuộc vào quá trình hồi phục của cơ thể chứ không phải do chính thiết bị; vì vậy việc sử dụng bề mặt tiếp xúc tốt hơn cũng chưa bao giờ giúp cải thiện kết quả điều trị.
Tổng kết trung thực về kết quả điều trị
Một nghiên cứu về phẫu thuật thay khớp ở các khớp ngón tay và ngón cái đã nêu rõ: phương pháp này thường mang lại hiệu quả giảm đau và mức độ hài lòng cao; tuy nhiên trước đây tỷ lệ biến chứng khá cao. Dù các vật liệu như kim loại–nhựa và pyrocarbon đã có nhiều tiến bộ, tỷ lệ sống sót của implant và tỷ lệ phải phẫu thuật lại vẫn là vấn đề đáng lo ngại [2].

Đối với bệnh viêm khớp thoái hóa, bằng chứng khoa học còn hạn chế
Phần lớn các tài liệu nghiên cứu đều tập trung vào bệnh viêm khớp dạng thấp, bởi vì đây là tình trạng dẫn đến những biến dạng khớp rõ rệt nhất cần được chỉnh sửa. Đối với viêm khớp thoái hóa, bằng chứng lại ít hơn nhiều: các dữ liệu hiện có chủ yếu là từ các nghiên cứu hồi cứu với mẫu nghiên cứu nhỏ và thời gian theo dõi tương đối ngắn [3].
Đáng chú ý là khi các tác giả ưu tiên sử dụng vật liệu pyrocarbon thay vì silicone trong điều trị viêm khớp thoái hóa, lý do đưa ra là khả năng cải thiện độ ổn định và sự cân đối của khớp; tuy nhiên các dữ liệu này chỉ mang tính sơ bộ mà thôi [3]. Đó là một quan điểm hợp lý, nhưng cần hiểu rằng đó là kết luận dựa trên cơ chế sinh lý học chứ không phải dựa trên bằng chứng thực tế chứng minh sự vượt trội của phương pháp này.
Độ bền vững đang trở thành một vấn đề đáng lo ngại
Mối lo ngại được nêu ra gần hai thập kỷ trước hiện nay ngày càng trở nên nghiêm trọng thay vì được giải quyết: độ bền vững của các implant càng trở nên quan trọng khi tuổi thọ con người tăng lên, và khi các ca phẫu thuật này được thực hiện trên những bệnh nhân trẻ tuổi, năng động hơn – những người sẽ sử dụng implant trong thời gian dài [4]. Trên thực tế, đây là lý do để bệnh nhân cần xác định rõ mục đích của ca phẫu thuật. Đối với việc giảm đau, chỉnh sửa dị dạng và cải thiện chức năng bàn tay trong những trường hợp có mức độ vận động vừa phải, đây là phương pháp phẫu thuật được hỗ trợ tốt và có lịch sử áp dụng lâu dài. Tuy nhiên, nếu muốn một phương pháp tái tạo có độ bền vững để chịu được việc sử dụng liên tục trong nhiều thập kỷ, thì đây không phải là lựa chọn thích hợp.

Classification

- Arthrodesis has been the gold standard in treating isolated end-stage MP arthritis, whether inflammatory or post-traumatic arthropathies [32].
- Arthrodesis is able to provide effective pain relief as well as restore stability to the joint, even in the setting of severe arthritis [32].
- The key to why MP fusion in the thumb is so successful lies in the relatively negligible loss of motion that results from MP fusion [32].
- By fusing the MP joint between 20 and 40° of flexion, stress can be relieved from the CMC joint by minimizing the activity required at the CMC for thumb opposition [32].

- For the Neuflex silicone implant, survivorship at 7 years was 88% when revision was the end point, but dropped to 68% when implant fracture was the end point [56].
- In a study of pyrocarbon arthroplasty for rheumatoid arthritis, complication and overall reoperation rates were high, with 1 in 10 patients undergoing revision within 5 years postoperatively [57].
