Cơn đau và tâm trạng luôn đi đôi với nhau. Bất kỳ ai đã từng trải qua một tuần tồi tệ và nhận thấy cơn đau vai trở nên dữ dội hơn đều hiểu điều này. Điều đó không có nghĩa là cơn đau chỉ là do tưởng tượng, cũng không có nghĩa là lỗi thuộc về bạn. Điều đó có nghĩa là hệ thần kinh truyền tải cảm giác đau và phần não bộ chịu trách nhiệm về cảm xúc lo âu thực chất là một hệ thống duy nhất; vì vậy việc tìm hiểu cách chúng tương tác với nhau trước khi phẫu thuật là điều rất cần thiết, thay vì phải phát hiện ra điều đó sau khi phẫu thuật.

Tác động của căng thẳng lên cảm giác đau

Hãy hình dung hệ thần kinh của bạn như một chiếc chuông báo động. Nhiệm vụ của nó là báo cho bạn biết khi nào có điều gì đó không ổn. Ở một cơ thể được nghỉ ngơi đầy đủ và trong trạng thái bình thường, chiếc chuông này nhạy cảm nhưng vẫn chính xác: chỉ khi có chấn thương thực sự thì nó mới kích hoạt, còn các chuyển động thông thường thì không.

Tuy nhiên, khi bạn phải chịu đau đớn trong nhiều tháng, ngủ không đủ giấc hoặc phải chịu nhiều áp lực ở nhà hay nơi làm việc, chiếc chuông báo động này sẽ trở nên nhạy cảm hơn. Nó bắt đầu kích hoạt ngay cả khi không có nguy hiểm nào: chỉ cần một cái chạm nhẹ, một chuyển động nhỏ, hoặc sức nặng của chiếc chăn cũng đủ khiến nó reo lên. Cơn đau là có thật, nhưng mức độ đau lúc này đã vượt xa mức độ tổn thương thực tế. Các bác sĩ gọi hiện tượng này là “nhạy cảm hóa trung ương”. Nói một cách đơn giản, các mạch thần kinh đã trở nên quá nhạy cảm; sự lo âu, giấc ngủ kém và nỗi sợ hãi lại càng làm tình trạng này trầm trọng thêm.

Đây là lý do tại sao hai bệnh nhân cùng trải qua một ca phẫu thuật giống hệt nhau lại có những tuần đầu hồi phục rất khác nhau. Phần mô được phục hồi là như nhau; nhưng “cài đặt” của hệ thần kinh báo động thì không giống nhau.

Tại sao điều này quan trọng đối với ca phẫu thuật của bạn

Nếu bạn đang gặp phải tình trạng lo âu hoặc tâm trạng chán nản, quá trình hồi phục của bạn có lẽ sẽ diễn ra khác so với những gì được mô tả trong sách giáo khoa. Có khả năng bạn sẽ cảm thấy đau nhiều hơn trong những tuần đầu sau phẫu thuật; các điểm số trên các bảng câu hỏi mà chúng tôi sử dụng cũng sẽ tăng chậm hơn. Tuy nhiên, bạn vẫn sẽ nhận được phần lớn lợi ích từ ca phẫu thuật; chỉ là bạn sẽ cảm nhận được điều đó muộn hơn và có thể cảm thấy ít chắc chắn về kết quả hơn trong quá trình hồi phục.

Có hai vấn đề cụ thể cần được đề cập:

  • Nỗi sợ khi vận động cánh tay là yếu tố có khả năng cản trở quá trình hồi phục nhiều nhất. Điều này hoàn toàn dễ hiểu: cánh tay bị đau, vừa trải qua phẫu thuật, nên việc cử động nó dường như sẽ gây hại. Tuy nhiên, nếu không vận động, vùng vừa được phẫu thuật sẽ bị cứng lại; tình trạng cứng khớp này khó khắc phục hơn nhiều so với việc ngăn chặn nó ngay từ đầu. Các bài tập mà bạn được hướng dẫn thực hiện chính là phương pháp điều trị, chứ không phải những hoạt động tùy chọn.
  • Những nỗi lo xuất hiện sau phẫu thuật cũng là điều bạn nên chia sẻ với chúng tôi. Những người bắt đầu cảm thấy lo âu hoặc chán nản trong vài tuần sau ca phẫu thuật vai có nguy cơ phải nhập viện trở lại, thường là do những cơn đau bất ngờ. Việc nhận thức được điều này và có kế hoạch ứng phó chính là chìa khóa để giải quyết vấn đề.

Tất cả những điều trên không làm thay đổi quyết định có nên phẫu thuật hay không; chúng chỉ giúp chúng tôi chuẩn bị tốt hơn cho bạn trước ca phẫu thuật.

Thói quen hữu ích nhất: vận động nhẹ nhàng mỗi ngày

Các bài tập do bác sĩ phẫu thuật hoặc chuyên viên trị liệu tay hướng dẫn thường rất đơn giản và có phần nhàm chán. Mục đích của chúng là để bạn thực hiện nhiều lần mỗi ngày, trong phạm vi giới hạn đã được chỉ định, dù bạn có muốn làm hay không. Dưới đây là một vài mẹo giúp việc tập luyện dễ dàng hơn:

  • Thực hiện theo giờ quy định, chứ không phải theo cảm giác của bản thân. Hãy gắn các bài tập này với những thói quen đã có sẵn: sau mỗi bữa ăn, hoặc mỗi khi pha trà.
  • Hãy chuẩn bị tinh thần cho cảm giác khó chịu và biết phân biệt chúng. Việc cảm thấy căng cứng rồi dần dịu đi khi thực hiện bài tập là bình thường. Ngược lại, nếu có cơn đau dữ dội ngày càng tăng, hoặc đau kéo dài nhiều giờ sau khi tập, thì đó là dấu hiệu bất thường; lúc này bạn nên giảm cường độ tập và báo ngay cho chúng tôi.
  • Hãy tưởng tượng các chuyển động trong đầu. Việc ngồi yên và hình dung cổ tay mình uốn cong một cách chi tiết nhiều lần mỗi ngày có vẻ kỳ lạ, nhưng các nghiên cứu đã chứng minh rằng phương pháp này giúp cải thiện khả năng vận động, sức nắm và chức năng tay sau khi bị gãy cổ tay [10]. Khi khớp chưa thể vận động được, não bộ vẫn có thể “luyện tập” các chuyển động này. Hãy thử làm điều này trước mỗi lần tập.
  • Hãy vận động toàn thân. Hãy đi bộ mỗi ngày. Cánh tay bạn đang được treo cố định, nhưng cơ thể còn lại thì không.

Những điều đơn giản giúp ích

  • Chăm sóc giấc ngủ của bạn. Giấc ngủ kém và cơn đau có mối liên hệ mật thiết với nhau; đặc biệt sau phẫu thuật vai thì việc ngủ rất khó khăn. Trang giấc ngủ, cơn đau và quá trình hồi phục cung cấp những bước thực hiện cụ thể.
  • Sử dụng thuốc giảm đau đúng theo chỉ định trong những ngày đầu sau phẫu thuật, thay vì chờ đến khi cơn đau trở nên dữ dội mới dùng thuốc. Xem thêm cách kiểm soát cơn đau sau phẫu thuật.
  • Duy trì thói quen sinh hoạt đều đặn. Thức dậy vào giờ vẫn thường lệ, mặc quần áo, ăn uống đúng giờ. Những ngày thiếu trật tự sẽ khiến cơn đau chiếm lĩnh mọi khoảng trống trong tâm trí.
  • Nghe những bản nhạc yêu thích. Đây không phải là lời nói sáo rỗng. Trong một thử nghiệm ngẫu nhiên, những bệnh nhân được trị liệu bằng âm nhạc trong lúc phẫu thuật thay khớp vai cho thấy mức độ giảm đau và lo âu rõ rệt hơn so với những người không được áp dụng phương pháp này [11]. Việc đeo tai nghe tại phòng chờ hoặc ở nhà hoàn toàn không tốn kém gì.
  • Hít thở chậm rãi. Hít vào trong 4 giây, thở ra trong 6 giây; thực hiện vài phút mỗi lần, nhiều lần mỗi ngày và đặc biệt khi cơn đau bùng phát. Đây là cách nhanh nhất để làm dịu cơn đau.
  • Giữ mối quan hệ xã hội. Việc chỉ dùng được một tay khiến người ta dễ cảm thấy cô đơn. Hãy nhận lời những lời mời thăm hỏi hoặc giúp đỡ, và để mọi người hỗ trợ những việc đòi hỏi phải dùng cả hai tay.
  • Hạn chế rượu và caffeine, vì cả hai đều làm giấc ngủ tồi tệ hơn và khiến cảm giác lo âu gia tăng.
  • Đánh giá tiến triển theo từng tuần chứ không phải từng ngày. Quá trình hồi phục không diễn ra theo đường thẳng. Một ngày tồi tệ ở tuần thứ ba không có nghĩa là ca phẫu thuật thất bại.

Nếu bạn đang mắc chứng lo âu hoặc trầm cảm

Hãy cho chúng tôi biết. Đây là tình trạng khá phổ biến; nó không phải là lý do để tránh phẫu thuật, và việc biết được điều này giúp chúng tôi lên kế hoạch phù hợp.

  • Tiếp tục dùng thuốc. Đừng ngưng dùng thuốc chống trầm cảm hay thuốc chống lo âu trước khi phẫu thuật trừ khi bác sĩ kê đơn yêu cầu như vậy. Việc ngưng thuốc đột ngột sẽ khiến những ngày xung quanh ca phẫu thuật trở nên khó khăn hơn.
  • Chuẩn bị sự hỗ trợ cần thiết trước ngày mổ. Nếu bạn đang theo điều trị với chuyên gia tâm lý hoặc nhà tư vấn, hãy thông báo cho họ về ngày phẫu thuật. Nếu chưa có ai hỗ trợ và bạn muốn tìm người hỗ trợ, bác sĩ đa khoa có thể giúp sắp xếp.
  • Điều trị chứng lo âu cũng có lợi cho cánh tay. Ở những bệnh nhân mắc chứng lo âu hoặc trầm cảm phải phẫu thuật sửa chữa cơ chóp xoay, việc điều trị các vấn đề về tâm lý trong vài tuần xung quanh ca phẫu thuật không chỉ giúp giảm cảm giác khó chịu mà còn cải thiện khả năng vận động và chức năng của vai trong ba tháng đầu [12]. Tâm trí và cánh tay của bạn không phải là hai vấn đề riêng biệt.

Khi nào cần tìm sự giúp đỡ

Hãy gọi cho phòng khám nếu:

  • Cơn đau dữ dội hơn nhiều so với mức được dự đoán, hoặc không hề thuyên giảm vào thời điểm mà bác sĩ đã nói là sẽ hết đau
  • Bạn không thể thực hiện các bài tập vận động do cảm giác sợ hãi chứ không phải do đau đớn
  • Sau phẫu thuật, bạn cảm thấy lo âu hoặc buồn bã nhiều hơn mà tình trạng này không cải thiện

Hãy gặp bác sĩ đa khoa nếu cảm giác buồn bã hoặc lo lắng kéo dài hơn vài tuần, nếu bạn hoàn toàn mất ngủ, hoặc nếu bạn ngừng làm những việc mà mình vốn thích.

Advanced reading: the deeper science (optional)

Phần này dành cho những độc giả muốn tìm hiểu các bằng chứng khoa học thay vì chỉ tóm tắt. Các nghiên cứu về mối liên hệ giữa tâm lý và quá trình hồi phục sau phẫu thuật chỉnh hình đang phát triển nhanh chóng; thực tế thì tình hình phức tạp hơn và đầy hy vọng hơn so với quan niệm “những người hay lo lắng sẽ hồi phục kém”.

Sức khỏe tâm thần quyết định điểm xuất phát, chứ không chỉ điểm kết thúc

Ở những bệnh nhân phải phẫu thuật sửa chữa lại gân cơ vùng vai, tình trạng sức khỏe tâm thần trước phẫu thuật có mối tương quan mạnh nhất so với mọi yếu tố khác đối với các chỉ số đau, chức năng vận động và mức độ hài lòng ban đầu – ngay cả trước khi tiến hành bất kỳ ca phẫu thuật nào [1]. Điều này rất quan trọng khi diễn giải các kết quả sau phẫu thuật: nếu điểm số ban đầu thấp, thì việc điểm số cuối cùng cũng thấp hơn vẫn có thể phản ánh mức độ cải thiện tương đương.

Lo âu và trầm cảm làm thay đổi quỹ đạo hồi phục, nhưng không thay đổi kết quả cuối cùng

Một nghiên cứu theo dõi tiến triển sau phẫu thuật thay khớp háng cho thấy các triệu chứng lo âu và trầm cảm có liên quan đến kết quả hồi phục kém hơn; quỹ đạo hồi phục của những bệnh nhân này cũng khác biệt so với những người không có các triệu chứng đó, dù cả hai nhóm đều có sự cải thiện lâm sàng đáng kể [2]. Sau phẫu thuật ổn định xương bánh chè, những bệnh nhân mắc rối loạn trầm cảm nặng hoặc những người từng bị trầm cảm nhưng chưa từng được chẩn đoán đều có điểm số đánh giá sức khỏe kém hơn cả trước và sau phẫu thuật; tuy nhiên họ vẫn nhận được lợi ích rõ rệt từ ca mổ [3]. Hai năm sau phẫu thuật nội soi khớp háng, tiền sử mắc chứng lo âu hoặc trầm cảm luôn có liên quan đến kết quả hồi phục kém hơn theo đánh giá của bệnh nhân; ngược lại, khả năng phục hồi tinh thần chỉ có mối liên hệ yếu và không ổn định [4].

Mô hình lặp lại này thể hiện ở việc đường cong hồi phục bị dịch chuyển chứ không phải phẳng ra: lợi ích từ phẫu thuật là có thật, và nó được đo lường từ một mức khởi đầu thấp hơn.

Đây không phải là một quy tắc cố định

Hai nghiên cứu về chi trên đã bác bỏ quan điểm đơn giản hóa này. Sau phẫu thuật thay khớp vùng gốc ngón cái, tiền sử mắc chứng trầm cảm dường như không làm xấu đi các kết quả được bệnh nhân đánh giá [5]. Trong các ca phẫu thuật điều trị các dây thần kinh bị tổn thương gây đau đớn (khối u thần kinh) ở cánh tay, những bệnh nhân có điểm sức khỏe tâm thần thấp trước phẫu thuật vẫn cho thấy mức độ giảm đau tương đương hoặc tốt hơn, cùng với sự cải thiện chức năng tương tự so với những bệnh nhân khác [6]. Tâm trạng chỉ là một yếu tố, chứ không phải là yếu tố quyết định mọi kết quả.

Yếu tố có thể thay đổi được là nỗi sợ vận động

Yếu tố tâm lý có ảnh hưởng cơ học rõ rệt nhất là chứng sợ vận động – tức là nỗi sợ rằng việc cử động sẽ gây tổn thương. Sau khi tiến hành thủ thuật ổn định khớp vai bằng phương pháp nội soi, chứng sợ vận động đã ảnh hưởng tiêu cực đến quá trình hồi phục chức năng và khả năng trở lại tập luyện thể thao của bệnh nhân [7]. Một đặc điểm liên quan là thói quen suy diễn tiêu cực về cơn đau (tức là luôn dự đoán điều tồi tệ nhất khi bị đau); đặc điểm này có khả năng dự báo mức độ hài lòng của bệnh nhân sau sáu tháng phẫu thuật cột sống [8]. Trong những tuần đầu sau khi phẫu thuật ổn định khớp vai bằng phương pháp nội soi, những bệnh nhân xuất hiện triệu chứng lo âu hoặc trầm cảm mới có nguy cơ phải nhập viện lại trong vòng 90 ngày cao hơn đáng kể [9].

Những phát hiện này là cơ sở cho các lời khuyên được đưa ra trên trang này: hãy vận động đều đặn theo giờ giấc, chuẩn bị tinh thần cho cảm giác khó chịu, và hãy chia sẻ với chúng tôi về những nỗi lo lắng mới phát sinh thay vì giấu kín chúng.

Những phương pháp thực sự hiệu quả, đã được kiểm chứng kỹ lưỡng

Có ba nghiên cứu so sánh được thực hiện một cách ngẫu nhiên và đáng được lưu ý. Việc kết hợp kỹ thuật hình dung vận động (việc tưởng tượng có cấu trúc về các chuyển động cần thực hiện) vào phác đồ điều trị tiêu chuẩn sau gãy xương quay xa giúp cải thiện chức năng vận động, khả năng duỗi cổ tay và sức mạnh nắm tay so với chỉ điều trị theo phác đồ tiêu chuẩn đơn thuần [10]. Liệu pháp âm nhạc trong quá trình phẫu thuật thay khớp vai giúp giảm đáng kể mức độ đau và lo âu so với phương pháp chăm sóc thông thường [11]. Đối với những bệnh nhân mắc chứng lo âu hoặc trầm cảm phải phẫu thuật sửa chữa gân cơ chóp xoay, việc sử dụng duloxetine – một thuốc chống trầm cảm cũng được dùng để giảm đau – giúp cải thiện tình trạng lo âu, mức độ đau đớn, phạm vi vận động và các chỉ số chức năng trong vòng ba tháng đầu; tuy nhiên có khoảng 1/6 bệnh nhân gặp phải triệu chứng buồn nôn hoặc nôn mửa [12].

Không phương pháp nào trong số này có thể chữa khỏi hoàn toàn; riêng trường hợp dùng duloxetine là quyết định kê đơn thuộc thẩm quyền của bác sĩ đa khoa hoặc bác sĩ tâm thần, không phải thứ mà người bệnh tự ý sử dụng. Tuy nhiên, cả ba nghiên cứu này đều cho thấy một điều chung: tâm trí không phải là yếu tố thụ động trong quá trình hồi phục, và chúng ta hoàn toàn có thể tác động lên nó.

Lưu ý về các bảng câu hỏi

Trước khi phẫu thuật, bạn có thể được yêu cầu điền vào một bảng câu hỏi ngắn về sức khỏe tâm thần. Đây không phải là bài kiểm tra mà bạn có thể trượt hay đỗ. Một nghiên cứu so sánh các công cụ sàng lọc trước khi phẫu thuật thay khớp cho thấy rằng thang điểm sức khỏe tâm thần tiêu chuẩn ngắn gọn này có hiệu quả tốt hơn so với bảng câu hỏi về khả năng phục hồi trong việc xác định những bệnh nhân có nguy cơ đạt kết quả điều trị kém [13]. Mục đích của việc này là để lên kế hoạch hỗ trợ phù hợp, chứ không phải để quyết định ai được phẫu thuật.
