How is your shoulder, in numbers?
The SPADI — Shoulder Pain and Disability Index — is a validated 13-question score of shoulder pain and function, used in research and clinics worldwide. Answer for the past week; your result is calculated instantly and never leaves your browser. Print it, or bring the numbers to your GP or specialist appointment — a score over time is far more useful than a score once.

Scores run from 0 (no pain or difficulty) to 100 (the worst). There is no single cut-off that means treatment is needed — the score is a snapshot of how your shoulder is affecting you this week. A change of around 8–13 points over time is generally considered a noticeable difference, which is why repeating the score — before and after physiotherapy, an injection or surgery — is more informative than a single number.
If your shoulder is limiting sleep, work or the things you care about, the right first step is your GP, who can examine you, arrange imaging where needed, and refer on if specialist review is warranted. Bring this score with you.
This questionnaire is an educational self-assessment, not a diagnosis, and does not replace a clinical assessment. Nothing you enter is transmitted or stored — the score is calculated on your device.
