Dị tật boutonnière làm thay đổi cách uốn cong của một ngón tay. Khớp giữa của ngón tay vẫn bị gập xuống, trong khi khớp cuối lại hướng lên thay vì gập lại. Tình trạng này xảy ra khi một gân nhỏ, dẹt nằm ở mặt sau ngón tay (gọi là dây chằng trung tâm) ngừng thực hiện chức năng của mình. Gân này thường có nhiệm vụ duỗi thẳng khớp giữa. Khi gân bị rách, giãn hoặc mòn, ngón tay sẽ không thể duỗi thẳng ở chỗ đó nữa.

Ngón tay có thể gây đau đớn và khiến việc sử dụng tay trở nên khó khăn hơn nhiều. Những thao tác đòi hỏi ngón tay phải thẳng sẽ trở nên khó thực hiện. Việc đeo găng tay, gõ bàn phím, cầm bút hoặc nhặt đồng xu đều có thể gây cảm giác khó chịu hoặc bất tiện. Tình trạng sưng và đau thường tuân theo một quy luật nhất định: vào buổi sáng khi vừa thức dậy, khớp có thể cứng và đau; sau đó dần trở nên linh hoạt hơn khi bắt đầu vận động. Sau một ngày làm việc vất vả, ngón tay có thể đau nhức và gập nhiều hơn bình thường. Cơn đau âm ỉ về đêm ở khớp cũng rất phổ biến khi ngón tay đã phải hoạt động quá mức.

Một điều cần lưu ý: việc phát hiện sớm là rất quan trọng. Nếu chấn thương dây chằng trung tâm được điều trị ngay sau khi xảy ra, dị tật thường có thể được ngăn chặn hoàn toàn.

Điều gì đang thực sự xảy ra

Hãy hình dung gân ở mặt sau ngón tay như một sợi dây chạy dọc theo phía trên ngón tay và chia thành nhiều nhánh gần khớp giữa. Một nhánh gọi là sợi gân trung tâm, gắn vào xương nằm giữa ngón tay và giúp khớp này duỗi thẳng ra. Hai nhánh còn lại chạy dọc hai bên sợi gân trung tâm. Bình thường, cả ba nhánh này cùng phối hợp hoạt động; chúng được giữ cố định phía trên khớp bởi những lớp mô mỏng, giống như các sợi dây cố định cột trụ lều đứng thẳng.

Khi ngón tay bị chấn thương hoặc bệnh viêm khớp dạng thấp làm mòn gân bên trong khớp, sợi gân trung tâm có thể bị rách hoặc giãn ra. Một khi đã bị đứt, hai nhánh bên sẽ trượt xuống phía dưới khớp thay vì chạy phía trên. Từ đó, chúng kéo khớp giữa ngón tay về vị trí cong thay vì giữ nó thẳng. Đó là lý do tại sao ngón tay bị cong ở khớp giữa và đầu ngón tay hướng lên trên.

Để tình trạng này xảy ra, cần có nhiều bộ phận bị tổn thương cùng lúc: sợi gân trung tâm, các lớp mô giữ các nhánh gân cố định, cùng một số sợi mô xung quanh. Chỉ khi tất cả những thành phần này đều bị ảnh hưởng thì các nhánh gân mới thực sự trượt khỏi vị trí ban đầu. Đây cũng là lý do vì sao ngón tay thường có cảm giác “bật” hoặc “vướng” ở khớp giữa trong giai đoạn đầu, trước khi dị dạng hoàn toàn xuất hiện.

Một điều nữa cần lưu ý: Khi dị dạng tồn tại trong thời gian dài, khớp sẽ dần cứng lại và thích nghi với tư thế cong đó. Ngay cả sau khi điều trị, vẫn có thể còn lại một vài độ mất khả năng duỗi thẳng ở khớp giữa. Đó là lý do tại sao việc điều trị sớm là vô cùng quan trọng, như đã giải thích ở phần trên.

Những biện pháp chúng tôi có thể áp dụng

Bác sĩ Kieran Hirpara, bác sĩ phẫu thuật chi trên tại Bệnh viện Mater Private Rockhampton, sẽ bắt đầu bằng các phương pháp ít xâm lấn nhất phù hợp với tình trạng của bạn. Thông thường, bệnh nhân được bác sĩ đa khoa giới thiệu đến phòng khám chúng tôi; nếu nhà vật lý trị liệu khuyên bạn đến gặp chúng tôi, bạn vẫn cần có giấy giới thiệu từ bác sĩ đa khoa để được hưởng mức hoàn trả chi phí từ Medicare. Đối với các dị dạng đã tồn tại từ lâu, chúng tôi thường bắt đầu bằng việc sử dụng nẹp. Sau khi ngón tay đã được giữ thẳng đủ lâu, bạn sẽ đeo nẹp giữ ngón tay ở tư thế cho phép tiếp tục sử dụng tay. Thời gian đeo nẹp là 3 tháng. Phương pháp này hiệu quả khi việc đeo nẹp ban đầu đã giúp khớp giữa giảm độ gập xuống dưới -20°, nghĩa là ngón tay có thể duỗi thẳng vượt qua mức 20 độ so với trạng thái thẳng hoàn toàn; đồng thời bạn phải chủ động đeo nẹp trong suốt thời gian đó. Việc bắt đầu điều trị trong vòng 6 tuần kể từ khi chấn thương xảy ra sẽ giúp phục hồi khả năng vận động của ngón tay đáng kể. Đối với các chấn thương dây chằng trung tâm mới xảy ra, chúng tôi sẽ giữ khớp giữa của ngón tay ở trạng thái duỗi thẳng hoàn toàn trong 4 tuần, trong khi khớp cuối vẫn được vận động, sau đó dùng nẹp có lò xo thêm 2 tuần nữa. Trong phần lớn thời gian này, bạn vẫn có thể sử dụng tay để thực hiện các công việc nhẹ.

Khi việc đeo nẹp không giúp ngón tay duỗi thẳng đủ mức, phẫu thuật là bước tiếp theo. Chúng tôi chỉ chỉ định phẫu thuật khi dây chằng đã bị tổn thương vượt quá khả năng điều chỉnh bằng nẹp, hoặc khi dị dạng đã cố định không thể thay đổi. Những điều bạn có thể mong đợi

Diễn biến của bệnh phụ thuộc rất nhiều vào tốc độ điều trị. Nếu chấn thương dây chằng giữa ngón tay mới xảy ra được cố định bằng nẹp sớm, người ta thường có thể ngăn chặn hoàn toàn sự hình thành dị dạng. Ngược lại, nếu không được điều trị, khớp giữa ngón tay thường sẽ bị cong và dần trở nên cố định ở tư thế đó. Khi dị dạng đã tồn tại trong thời gian dài, ngay cả các phương pháp điều trị tốt cũng có thể khiến ngón tay không thể duỗi thẳng hoàn toàn mà chỉ đạt được mức góc duỗi thấp hơn một chút so với bình thường.

Đối với những trường hợp dị dạng đã hình thành, bước đầu tiên là sử dụng nẹp để duỗi thẳng khớp giữa; sau đó là đeo nẹp trong 3 tháng trong lúc vẫn tiếp tục sử dụng tay. Phương pháp này hiệu quả nếu nẹp có thể đưa góc duỗi của khớp xuống dưới mức -20°, tức là ngón tay duỗi thẳng vượt qua điểm cách mức hoàn toàn thẳng 20 độ. Trong suốt 3 tháng đó, bạn cần vận động ngón tay tích cực khi đeo nẹp để phục hồi khả năng duỗi thẳng. Đây là quá trình chậm rãi và đòi hỏi sự nỗ lực từ phía bạn: nẹp chỉ phát huy tác dụng nếu bạn thực sự vận động ngón tay khi đeo nó.

Khi việc đeo nẹp đã đạt đến giới hạn hiệu quả, phẫu thuật là bước tiếp theo. Trong trường hợp các sợi dây chằng bên bị mòn hoặc tổn thương, người ta có thể dùng một đoạn gân từ cổ tay của chính bệnh nhân để tái tạo lại chúng. Ở những người đã trải qua phẫu thuật này đối với dị dạng kéo dài, kết quả điều trị được đánh giá là tốt hoặc rất tốt ở 16/18 bệnh nhân. Một phương pháp khác dành cho dị dạng lâu năm là cắt bỏ dây chằng ở khớp cuối; phương pháp này giúp giảm đáng kể tình trạng khuyết tật và phục hồi khả năng duỗi thẳng ở khớp đó.

Cần lưu ý một vài điểm quan trọng. Ở giai đoạn muộn của dị dạng này, chỉ phẫu thuật mô mềm có thể không đảm bảo hiệu quả điều trị lâu dài. Nếu bạn mắc bệnh viêm khớp dạng thấp, dị dạng ngón tay có thể tiếp tục phát triển theo tiến triển của bệnh; vì vậy kế hoạch điều trị sẽ dựa trên mức độ cố định của khớp, khả năng vận động và tình trạng bề mặt khớp. Khi nào nên đi khám bác sĩ

Tại sao dị tật ngày càng nặng thay vì giữ nguyên trạng thái

Dị tật Boutonnière là một dạng dị tật tiến triển; điều này khá hiếm gặp và cần được hiểu rõ. Gân chính nối với phần gốc của xương giữa và có chức năng duỗi thẳng khớp giữa ngón tay. Dị dạng không phải dạng boutonnière

Ngón tay có thể giữ nguyên tư thế như vậy mà hoàn toàn không có chấn thương ở dây chằng trung tâm; các phương pháp điều trị cũng hoàn toàn khác nhau. Dị dạng boutonnière và dị dạng pseudoboutonnière khác nhau về cơ chế bệnh sinh, chẩn đoán và cách xử trí [2]. Dạng pseudoboutonnière xuất hiện do chấn thương ở màng liên khớp mu bàn tay tại khớp giữa; lúc này khớp ngón tay cuối vẫn có khả năng gập bình thường thay vì bị kéo ra trạng thái gập quá mức.

Anatomy & Pathophysiology

Extensor Mechanism Anatomy

  • The extensor mechanism of the finger at the level of the proximal interphalangeal (PIP) joint consists of both intrinsic and extrinsic contributions [19].
  • The extensor tendon entering the digit is the continuation of the extensor digitorum communis (EDC), with contributions from the extensor indicis proprius in the index finger and the extensor digiti quinti in the small finger [19].
  • In 30% of cases, the EDC has a separate insertion into the base of the proximal phalanx, but extension of the metacarpophalangeal (MP) joint most often occurs through the pull of the sagittal bands [19].
  • The deep head of each dorsal interosseous muscle forms a lateral tendon, or lateral band, at the level of the MP joint [25].
  • At the level of the middle of the proximal phalanx, transverse fibers arch dorsally from each lateral band to join over the dorsum of the finger, flexing the proximal phalanx [25].

Classification
